Why MDS Data Conflicts Happen

Your clinical staff is extremely busy caring for your residents.

Documenting your residents' care requires careful observation skills, charting time free of distractions, and strong familiarity with a lengthy and complex assessment instrument -- the MDS 2.0. 

Any of the following can result in errors and conflicts in MDS records: 

  • Data entry errors

  • MDS coding errors

  • Repeating responses from a previous assessment

  • Gaps in charting between shifts

  • Subjective judgments rather than objective observations 

Our Data Logic Reporting lists conflicts that occur in each MDS, and how many times particular data conflicts occur in range of MDS records. If a data conflict occurs frequently, this might indicate the need to strengthen assessment skills by providing more training for the staff, or arranging for less distraction during charting times, and better communication among staff members.
